Referrals & physician resources
We accept referrals from family physicians, specialists, urgent-care physicians, sports-medicine physicians and chiropractors throughout Guelph and surrounding communities.
How to refer
- 1Complete a signed requisition specifying the exam, clinical indication and any prior imaging.
- 2Send the requisition to our clinic by fax, encrypted email or directly with the patient.
- 3Our team contacts the patient within one business day to schedule.
- 4A written report is delivered to your office within 48 hours of the exam.
Accepted requisitions
- Frontier Diagnostics requisition forms (preferred)
- Provincial standard requisition forms with a valid signature
- Custom EMR-printed requisitions including patient identifiers, clinical history and the requested exam
Reports & follow-up
Reports are sent to the referring physician's preferred fax number or via secure EMR delivery within 48 hours. For urgent or unexpected findings, our radiologists will contact the referring office directly.
